Primary molar teeth which are lost prematurely can lead to significant problems with the alignment and position of the underlying permanent bicuspid and canine teeth. The most common reason for premature loss of primary molars is unrestorable caries (tooth decay). Generally, space maintainers are prescribed when primary molar teeth are lost too early.

Premature loss of primary anterior (front) teeth typically does not cause functional problems, and no replacement or space maintainer is required. If multiple anterior primary teeth are lost prematurely, and the child's appearance is of concern to the parents, a removable or fixed denture can be made for the child. Removable dentures are generally ill advised for children under the age of 3, because young children are usually incapable of understanding the reasons for wearing them or complying with their use.
